When You use the health record import Services to automatically import your health information to get Medicare Advantage plan recommendations, all PHI that You submit is used and disclosed by Golden Outlook Inc. as a Business Associate (as defined by HIPAA) according to the terms of a Business Associate Agreement between Us and the health information Provider. This means that Golden Outlook Inc. may only use and disclose Your PHI for plan recommendation services offered.

Managing Your Protected Health Information

Can You opt out of Golden Outlooks Inc.'s use and disclosure of Your Protected Health Information (PHI) or access, change, or remove Your Protected Health Information?

When You use the health record import services, You may receive various administrative e-mails automatically sent by Golden Outlook Inc. Golden Outlook Inc. treats as PHI all personally identifying information that You submit when you request to import your health records to our application. As described in the "How we use your Personal Information" section above, PHI is used and disclosed by Golden Outlook Inc. according to the terms of its Business Associate Agreement with health data providers. You are not required to share your PHI to use our applications. Until the termination of Our Business Associate Agreement with a particular health data provider, Golden Outlook Inc. can use and disclose Your PHI as described in the "How we use your Personal Information” section above following the termination of Your account with Golden Outlook Inc.
